FAI Trigger & Scope

  • Is a full or partial FAI required (new part, change in design, process, source, or 2-year lapse in production)? *
  • Part number *
  • Part revision level *
  • Drawing / model revision matches the FAI package? *

Form 1 — Part Number Accountability

  • Are all part and assembly numbers, revisions, and serial/lot numbers recorded on Form 1? *
  • Are all sub-assemblies and detail parts accounted for with their own FAI references? *
  • Is the reason for full/partial FAI documented? *

Form 2 — Materials, Processes & Special Requirements

  • Are raw material specifications and certifications recorded and traceable? *
  • Are special processes (heat treat, plating, NDT) listed with the approved special-process source? *
  • Are functional test and certificate-of-conformance references included? *

Form 3 — Characteristic Accountability

  • Are all design characteristics uniquely numbered (ballooned) and listed on Form 3? *
  • Is each characteristic's requirement, actual measured result, and inspection method recorded? *
  • Number of nonconforming characteristics *
  • Are any nonconformances documented with disposition/approval references? *

FAI Completion

  • Is the FAI complete with all designed characteristics verified as conforming (or dispositioned)? *
  • FAI completion date *
  • Quality reviewer signature *
