Sort (Seiri)
- Are only necessary items present in the work area, with unneeded items removed or red-tagged? *
- Is a red-tag process in place for questionable items?
- Are obsolete tools, materials, and documents removed? *
Set in Order (Seiton)
- Does every item have a designated and labeled location? *
- Are shadow boards, floor markings, or other visual controls used to indicate placement? *
- Can needed items be located and returned within seconds?
Shine (Seiso)
- Is the work area clean and free of dirt, debris, and leaks? *
- Are cleaning responsibilities and schedules assigned? *
- Is cleaning used as a form of inspection to detect abnormalities?
Standardize (Seiketsu)
- Are visual standards (photos, layouts, standard work) posted for the first three S's? *
- Are 5S responsibilities documented and assigned? *
Sustain (Shitsuke)
- Are regular 5S audits conducted and scored? *
- Are audit results posted and improvement actions tracked? *
- Does management participate in and reinforce 5S?
- Overall 5S score for this area
