Approved Ventilation Plan
- Is a current MSHA-approved ventilation plan on file and available at the mine? *
- Are ventilation controls (stoppings, overcasts, regulators, doors) installed and maintained as specified in the plan? *
- Is a mine ventilation map maintained showing air direction, quantities, and controls? *
Air Quantity & Velocity
- Is the minimum air quantity maintained at each working face as required by the approved plan? *
- Recorded air quantity at the last open crosscut or working face (cfm). *
- Is the mechanical ventilation system (main fans) equipped with pressure-recording and fan-stoppage warning devices? *
Methane & Air Quality Control
- Are methane tests conducted before and during work in accordance with the required frequency? *
- Is work stopped and personnel withdrawn when methane reaches 1.0% in a working place per regulation? *
- Are threshold limits for oxygen, carbon dioxide, and other contaminants maintained? *
Respirable Dust & Diesel
- Are respirable dust controls maintained in accordance with the approved plan? *
- Where diesel equipment is used underground, is the ventilation adequate to dilute diesel exhaust to permissible limits? *
- Are respirable dust samples collected and results maintained to verify compliance with the applicable exposure limit? *
Examinations & Records
- Are preshift and on-shift examinations of ventilation conducted by a certified person and recorded? *
- Are weekly examinations of seals, worked-out areas, and bleeder systems conducted? *
- Attach the most recent ventilation examination record.
