Dam Safety Review
- Has a periodic dam safety review been completed by a qualified independent engineer within the required interval? *
- Does the review assess stability against the current consequence classification and applicable design flood and seismic loads? *
- Is the current consequence/hazard classification of the facility documented and up to date? *
Surveillance & Monitoring
- Are instrumentation readings (piezometers, inclinometers, survey monuments) collected and reviewed against trigger action response levels? *
- Are routine surveillance inspections of the embankment, beach, and decant performed and documented? *
- Is the phreatic surface and pond freeboard maintained within design limits? *
Governance & Accountability
- Is an Engineer of Record and a Responsible Tailings Facility Engineer designated for the facility? *
- Is an operations, maintenance, and surveillance (OMS) manual current and being followed? *
- Are surveillance findings and any exceedances reported through a defined accountability chain? *
Emergency Preparedness Plan
- Is an emergency preparedness and response plan (EPP/EPRP) in place addressing credible failure modes and dam-breach inundation zones? *
- Has a dam-breach and inundation study been performed to define downstream evacuation areas? *
- Are emergency notification procedures coordinated with downstream communities and public authorities? *
- Are EPP drills and exercises conducted and documented? *
