Permitting & Authorization
- Does the well hold a valid Class II injection permit or authorization by rule from the primacy agency? *
- Has the injection zone and confining zone been evaluated to ensure containment of injected fluids? *
- Was an area-of-review conducted to identify wells that could serve as conduits for fluid migration? *
Mechanical Integrity
- Has a current mechanical integrity test (Part I internal and Part II external) demonstrated no significant leak or fluid movement? *
- Is the tubing-casing annulus filled with a corrosion-inhibiting fluid and monitored for pressure? *
- Record the date of the most recent mechanical integrity test. *
Operating Controls
- Is injection pressure maintained below the maximum authorized surface injection pressure to prevent fracturing the confining zone? *
- Are injection pressure, rate, and volume continuously monitored and recorded? *
- Is an automatic alarm or shutdown provided for pressure exceedance or annulus pressure change? *
Monitoring & Reporting
- Are monthly or quarterly injection volumes and pressures reported to the primacy agency as required? *
- Is the injected fluid characterized to confirm it is compatible with the permit and the injection formation? *
- Are records of monitoring, testing, and any exceedances retained for the required period? *
