MSHA Surface Mine Workplace Examination Checklist
Conduct and document the required examination of working places at a surface metal/nonmetal mine to identify hazardous conditions.

Examination Timing and Scope
- Is a competent person examining each working place at least once each shift before work begins or as miners begin work? *
- Name of the competent person who conducted the examination. *
- Date and shift of this examination. *
Ground and Highwall Conditions
- Are highwalls, benches, and ground conditions free of loose, unconsolidated, or hazardous material? *
- Are berms or guardrails in place along elevated roadways and dump points? *
- Are areas of loose or unstable ground barricaded or posted to prevent entry? *
Mobile Equipment and Traffic
- Are haul roads maintained and free of hazards for mobile equipment? *
- Do self-propelled mobile equipment have functional service brakes, backup alarms, and seatbelts as required? *
- Are windows and mirrors on mobile equipment maintained to provide adequate operator visibility? *
Guarding, Electrical and Housekeeping
- Are moving machine parts (gears, belts, pulleys) guarded to prevent contact? *
- Are electrical conductors and equipment free of damage and properly grounded? *
- Are work areas free of slip, trip, and fall hazards and accumulated debris? *
Corrective Action and Recordkeeping
- Were any adverse conditions found promptly corrected or the area posted/removed from service? *
- Does the examination record include the location, hazardous condition(s) found, and date of corrective action? *
- Are examination records retained for at least one year and available to MSHA and miners? *
- Attach a photo of any hazardous condition observed and its corrective action.
