Permit & Outfalls
- Does the operation hold a current NPDES permit covering all point-source discharge outfalls? *
- Are all permitted outfalls identified, mapped, and consistent with the permit application? *
- Are the applicable effluent limitation guidelines (coal 40 CFR 434 or ore mining 40 CFR 440) correctly applied to each discharge? *
Effluent Limits & Treatment
- Are discharges meeting permit limits for pH, total suspended solids, and metals (e.g., iron, manganese) applicable to the mine type? *
- Are treatment systems (settling ponds, chemical treatment, or passive systems) operating and maintained to meet limits? *
- Is flow measured and recorded at each monitored outfall? *
Monitoring & Reporting
- Is effluent sampling performed at the frequency, parameters, and methods specified in the permit? *
- Are discharge monitoring reports (DMRs) submitted on time to the permitting authority? *
- Are permit exceedances reported and corrective actions documented? *
Stormwater & Non-Point Controls
- Is a stormwater pollution prevention plan implemented for the site's industrial stormwater discharges? *
- Are sediment and erosion controls maintained to minimize contact water and runoff from disturbed areas? *
- Is contact water diverted or segregated from clean run-on where practicable? *
