Road Design & Berms
- Are berms or guardrails provided on elevated roadways where a drop-off exists that could cause injury? *
- Is berm mid-axle height at least the height of the largest vehicle's tire using the roadway? *
- Are road width, grade, and curve design suitable for the largest haulage equipment in use? *
- Are road surfaces maintained to control dust, water, and traction hazards? *
Traffic Controls & Right-of-Way
- Are traffic rules, right-of-way, and speed limits established and communicated to operators? *
- Are intersections, dump points, and blind areas controlled with signs, signals, or spotters? *
- Are light and heavy vehicle interactions managed with buffer zones or separation? *
Mobile Equipment Condition
- Are pre-operational (pre-shift) inspections of mobile equipment performed and defects reported? *
- Are service brakes and parking brakes tested and capable of holding the equipment on grade? *
- Are ROPS/FOPS, seat belts, backup alarms, and horns present and functional? *
- Are windshields, mirrors, cameras, and lighting adequate for operator visibility? *
Dump Points & Parking
- Are dump-point berms or bumper blocks provided and of adequate size? *
- Are procedures in place for chocking, blocking, or lowering equipment when parked or left unattended? *
- Are edge conditions at dump points inspected before dumping? *
