Approved Ground/Roof Control Plan
- Is an MSHA-approved roof control plan (coal) or ground control plan (metal/nonmetal) available at the mine? *
- Are the approved support spacing and installation sequences followed for the mining method in use? *
- Has the plan been reviewed and updated for changed geologic or mining conditions? *
Roof & Rib Support Installation
- Are roof bolts, mats, straps, or other support installed within the maximum allowable distance from the face? *
- Is support installed under permanent or temporary support protection (no work or travel under unsupported roof)? *
- Are roof bolt torque/anchorage tests conducted to verify support adequacy? *
- Are areas of unsupported roof posted or barricaded against entry? *
Scaling & Loose Ground
- Is loose or hazardous rock on the back, ribs, and face scaled down or supported before work proceeds? *
- Is scaling performed with tools and from a location that protects the miner? *
- Are persons prohibited from working or traveling beneath loose or unsupported ground until it is scaled or supported? *
Examination & Detection
- Are examinations for ground conditions (sound-and-vibration, visual) conducted at required intervals? *
- Is a program in place to detect and respond to changing roof conditions such as roof falls or ground movement? *
- Are hazardous conditions corrected before miners are exposed? *
