Advance Request and Authorization
- Did the vessel provide the required advance request for port entry with the minimum notice period? *
- Is the vessel's authorization to fish and relevant catch documentation verified before entry is granted? *
- Is the vessel checked against RFMO IUU vessel lists and any denial-of-entry records? *
Vessel Identification
- Is the vessel's flag state, IMO number (where applicable), and unique vessel identifier confirmed? *
- Is the automatic identification system (AIS) / vessel monitoring track reviewed for gaps or suspicious activity? *
- Has the vessel been party to any at-sea transshipment that is documented and authorized? *
Port Inspection
- Was a port inspection conducted in accordance with the required inspection procedures and minimum standards? *
- Do the species, quantities, and gear on board match the vessel's documentation and authorizations? *
- Date the port inspection was completed. *
Findings and Actions
- Where IUU fishing is suspected, is use of port (landing, transshipment, refueling, resupply) denied? *
- Are inspection results and any evidence of violations communicated to the flag State and relevant RFMO? *
- Attach the completed port inspection report.
