Cyanide Handling & Storage
- Is sodium cyanide received, mixed, and stored using procedures that minimize worker exposure and releases? *
- Are unloading and mixing areas equipped with secondary containment and spill response equipment? *
- Is process solution pH maintained in the alkaline range to prevent HCN gas evolution? *
- Are cyanide antidote kits, HCN gas monitors, and trained responders available? *
Leach Pad & Solution Containment
- Are the leach pad, ponds, and solution channels lined with an engineered liner system and leak collection? *
- Is adequate freeboard maintained in pregnant, barren, and overflow (event) ponds? *
- Is a leak detection and recovery system monitored between primary and secondary liners? *
- Record the current pregnant solution pond freeboard (ft). *
Wildlife & Environmental Protection
- Are open waters containing cyanide maintained below the concentration protective of wildlife, or protected by exclusion measures? *
- Is weak-acid-dissociable (WAD) cyanide monitored in solutions and open waters? *
- Are wildlife mortality events recorded, investigated, and reported? *
Decommissioning & Emergency Response
- Is a cyanide-specific emergency response plan in place and coordinated with local responders and medical facilities? *
- Are heap and pond solution rinse/detoxification procedures documented for closure? *
- Are transportation and chain-of-custody records for cyanide shipments maintained? *
