Study Preparation
- Is the HAZOP based on current, verified P&IDs and process flow diagrams reflecting as-built conditions? *
- Is the multidisciplinary study team composed of operations, process, instrumentation, and safety expertise led by a trained facilitator? *
- Have the process nodes and study boundaries been defined and documented before the sessions? *
Deviation Analysis
- Were standard guidewords (no, more, less, reverse, as-well-as) systematically applied to each parameter for every node? *
- Were high-hazard scenarios such as loss of containment, overpressure, and cryogenic release specifically examined? *
- Were causes and consequences documented for each credible deviation? *
Safeguards & Risk Ranking
- Were existing safeguards (relief devices, SIS, alarms, procedures) identified for each hazardous deviation? *
- Was residual risk ranked using a defined risk matrix to prioritize recommendations? *
- Were independent protection layers assessed for scenarios requiring further review (LOPA)?
Recommendations & Follow-Up
- Are all recommendations assigned an owner and a target completion date? *
- Is a tracking system in place to document resolution and closure of each action item? *
- Is the HAZOP scheduled for revalidation at least every five years or upon significant process change? *
