Chemical Disclosure & FracFocus Registry

  • Was a chemical disclosure registry form submitted to FracFocus within the state-required window after completion of the fracturing operation? *
  • Does the disclosure list the total volume of base fluid (water) and each additive by trade name, supplier, and purpose? *
  • Are the CAS numbers and maximum concentrations of each hazardous chemical component disclosed? *
  • For any chemicals withheld as trade secret, is a claim documented with a supporting justification and a generic family name provided? *
  • Date the FracFocus disclosure was submitted. *

Water Sourcing & Management

  • Is the source of base fluid (fresh, brackish, recycled, or produced water) documented with applicable water withdrawal permits? *
  • Are on-site fluid storage impoundments or tanks lined and equipped with secondary containment? *
  • Are freeboard and leak-detection requirements for storage pits or tanks maintained and inspected? *

Well Integrity & Pressure Barriers

  • Has surface casing been set and cemented across all usable-quality water zones before fracturing? *
  • Was a mechanical integrity test (MIT) or casing/pressure test performed and passed prior to the stimulation treatment? *
  • Is the maximum anticipated treating pressure below the tested casing and wellhead pressure ratings? *
  • Is annular pressure continuously monitored during the treatment for early detection of a barrier failure? *

Flowback & Produced Water Handling

  • Is flowback and produced fluid collected in tanks or lined pits and characterized before disposal or reuse? *
  • Is fluid disposed via a permitted Class II UIC injection well, recycling, or other approved method? *
  • Are NORM (naturally occurring radioactive material) screening and handling procedures applied to flowback and solids? *

Notification & Recordkeeping

  • Was pre-fracturing notification provided to the state regulatory agency and, where required, to nearby water-well owners? *
  • Are pre- and post-fracturing baseline water sampling records retained where required by state rule?
  • Attach the FracFocus disclosure confirmation and completion report.
