SEMS Program Documentation

Core program and general requirements.

  • Is a documented Safety and Environmental Management System (SEMS) program maintained and available at the facility? *
  • Does the SEMS program address all facilities and activities on the Outer Continental Shelf under the operator's control? *
  • Has management designated the person(s) responsible for developing and implementing the SEMS program? *
  • Date of the most recent SEMS program review or update. *

Safety and Environmental Information & Hazards Analysis

  • Is complete safety and environmental information compiled for the facility process, mechanical, and facility design? *
  • Has a facility-level hazards analysis been performed and documented for each facility? *
  • Are identified hazards addressed with recommendations that are resolved and tracked to completion? *

Management of Change and Operating Procedures

  • Are written management of change (MOC) procedures in place for changes to facilities, equipment, and personnel? *
  • Are written operating procedures developed, maintained, and accessible to affected employees? *
  • Do operating procedures address startup, normal operations, temporary operations, emergency shutdown, and normal shutdown? *

Safe Work Practices and Training

  • Are safe work practices (e.g., hot work, lockout/tagout, confined space) documented and enforced, including for contractors? *
  • Are contractor safety performance and selection criteria documented? *
  • Have all personnel received training appropriate to their assigned duties before performing work? *

Mechanical Integrity and Pre-Startup Review

  • Is a mechanical integrity program implemented that ensures critical equipment is designed, installed, tested, and maintained? *
  • Is a pre-startup safety and environmental review (PSSR) conducted for new and significantly modified facilities? *
  • Are deficiencies identified during mechanical integrity inspection and testing corrected before equipment is returned to service? *

Incident Investigation, Emergency Response & SWA

  • Are incidents (including near-misses) investigated, with findings and corrective actions documented and tracked? *
  • Are emergency response and control plans validated and drills conducted? *
  • Does every worker have documented Stop Work Authority (SWA) and Ultimate Work Authority (UWA) designated for the facility? *
  • Attach the current SEMS audit report or self-inspection record.
