Survival Craft
- Are all lifeboats and liferafts present in the required numbers with sufficient capacity for all persons on board? *
- Is each liferaft serviced within the last 12 months (or approved extended interval) with a valid servicing certificate on board? *
- Enter the date of the last liferaft servicing. *
- Are hydrostatic release units (HRUs) fitted, in date, and correctly rigged on float-free liferafts? *
- Are lifeboat engines starting readily and running ahead and astern satisfactorily? *
- Is the equipment inventory in each survival craft complete and in serviceable condition? *
Launching Appliances and Release Gear
- Are davits, winches, falls, and blocks free of corrosion, deformation, and lubricated for smooth operation? *
- Have lifeboat falls been renewed or turned end-for-end within the required interval (renewal not exceeding 5 years)? *
- Are on-load release mechanisms compliant with the LSA Code and free from unintentional-release risk? *
- Is the winch brake tested and holding correctly at the required load? *
Personal Life-Saving Appliances
- Is a lifejacket provided for every person on board plus the required additional stowage for watchkeeping and survival craft? *
- Are immersion suits or anti-exposure suits provided in the required numbers and within service dates? *
- Are lifebuoys provided in the correct number and distribution, with lights, smoke signals, and buoyant lines as required? *
- Are lifejacket lights and self-igniting lights on lifebuoys operational with in-date batteries? *
Distress and Communication Equipment
- Are the required rocket parachute flares, hand flares, and buoyant smoke signals on board and within expiry? *
- Enter the earliest expiry date among pyrotechnic distress signals. *
- Are two-way VHF radiotelephone apparatus (survival craft portable radios) available and operational in the required numbers? *
- Are radar transponders (SARTs) or AIS-SARTs fitted, operational, and correctly stowed for rapid transfer to survival craft? *
Muster, Drills and Records
- Is the muster list posted, current, and consistent with the crew list and emergency duties? *
- Have abandon-ship and fire drills been conducted at the required intervals and recorded in the log? *
- Has each lifeboat been launched and manoeuvred in the water within the last 3 months? *
- Attach a photo of the current muster list at the muster station.
