Mooring Equipment Condition
- Are mooring lines within their retirement criteria and free from significant wear, cuts, or damage? *
- Are winches, drums, brakes, and their controls maintained and functioning correctly? *
- Have winch brake holding capacities been tested and recorded within the required interval? *
Fittings and Documentation
- Are bollards, fairleads, rollers, and chocks free from sharp edges, cracks, and excessive wear? *
- Is a Mooring System Management Plan (MSMP) and Line Management Plan available and followed? *
- Are Safe Working Loads (SWL/MBL) of fittings and lines documented and known to the crew? *
Operation and Manning
- Is the mooring station adequately manned by trained personnel under a responsible officer? *
- Are snap-back and danger zones identified and personnel kept clear of them during operations? *
- Is appropriate PPE worn and communication maintained between stations and the bridge? *
Records and Review
- Are mooring line inspection and replacement records maintained? *
- Are mooring-related incidents or near-misses reported and reviewed for corrective action? *
