Enclosed Space Entry Permit & Rescue Readiness
A permit-to-work checklist for safe entry into enclosed spaces, covering atmosphere testing, ventilation, and rescue arrangements per SOLAS and IMO guidance.

Authorization and Assessment
- Has a risk assessment been completed and an entry permit authorized by a responsible officer? *
- Enter the name of the responsible officer authorizing the entry. *
- Has the space been identified and isolated from hazards (electrically, mechanically, from cargo/piping systems)? *
Atmosphere Testing
- Has the atmosphere been tested with a calibrated multi-gas meter before entry? *
- Enter the measured oxygen content (percent by volume). *
- Is the flammable gas / hydrocarbon reading below 1% of the Lower Flammable Limit (LFL)? *
- Are toxic gas readings (H2S, CO, etc.) within safe limits for the space? *
- Is continuous or periodic atmosphere monitoring arranged for the duration of the entry? *
Ventilation and Access
- Has the space been mechanically ventilated before and continuously during entry? *
- Is access adequate and lighting sufficient for safe entry and egress? *
- Is the equipment to be taken into the space (lighting, tools) of a safe type suitable for the atmosphere? *
Communication, Standby and Rescue
- Is a means of communication established between entrants and the attendant at the entrance? *
- Is a trained standby person stationed at the entrance for the duration of the entry? *
- Are rescue and resuscitation equipment, including breathing apparatus and lifelines, available at the entrance? *
- Has an enclosed space entry and rescue drill been conducted within the last 2 months? *
