- Are board meetings noticed and conducted under state open-meetings law? *
- Is the annual state library report filed on time? *
Patron Privacy & Records
- Are patron circulation and borrowing records kept confidential under state library privacy law? *
- Is patron information released only with proper legal process (subpoena/court order)? *
- Are records retained no longer than necessary and securely destroyed? *
Internet Safety & CIPA
- If receiving E-rate/LSTA funds, does the library enforce a technology protection measure (filter)? *
- Has the required internet safety policy public hearing been held and policy adopted? *
- Can filtering be disabled for adults engaged in bona fide research on request? *
Accessibility & Services
- Is the facility and website accessible to patrons with disabilities? *
- Are programs and materials offered in formats/languages serving the community? *
- Are staff trained on serving patrons experiencing crises or homelessness? *
Finance & Stewardship
- Are gift, grant, and endowment funds tracked separately and used per donor restrictions? *
