Administrative Requirements
- For entities with 50 or more employees, is a responsible ADA Coordinator designated? *
- Is a grievance procedure for ADA complaints adopted and published? *
- Has a self-evaluation of services, policies, and practices been completed and retained? *
- Is a public notice of ADA rights and nondiscrimination provided? *
Program Accessibility
- Are programs and services, when viewed in their entirety, readily accessible to and usable by individuals with disabilities? *
- Does new construction and alteration comply with the 2010 ADA Standards for Accessible Design? *
- For existing facilities, is a transition plan in place to remove access barriers? *
Effective Communication
- Are appropriate auxiliary aids and services provided to ensure effective communication? *
- Is primary consideration given to the aid or service requested by the individual with a disability? *
- Are telephone and relay-service accommodations (e.g., TTY/711) available? *
Reasonable Modifications & Service Animals
- Are reasonable modifications to policies made unless they would fundamentally alter the program? *
- Are service animals permitted in all areas open to the public? *
- Is surcharging individuals with disabilities to cover accessibility measures prohibited? *
Web & Digital Accessibility
- Do the entity's web content and mobile apps conform to WCAG 2.1 Level AA? *
- Is the applicable compliance deadline tracked based on population size? *
- Is a mechanism provided for users to report inaccessible digital content and request an accessible alternative? *
