Surface Condition & Housekeeping
- Are walking-working surfaces maintained clean, dry, orderly, and in good repair? *
- Are surfaces capable of supporting the maximum intended loads? *
- Is safe access and egress provided to and from all walking-working surfaces? *
Fall Hazards & Protection
- Is fall protection provided at unprotected sides and edges 4 feet or more above a lower level? *
- Are holes (including skylights) 4 feet or more above a lower level guarded by covers or rails? *
- Are guardrail systems at 42 inches (plus/minus 3) with midrails and able to withstand 200 lbf? *
- Are personal fall arrest systems inspected before each use and anchored properly? *
Ladders & Stairways
- Do fixed ladders over 24 feet have fall protection (cage/well legacy or ladder safety systems)? *
- Are stairways with 4 or more risers provided with standard handrails? *
- Are ladders inspected periodically and defective ladders tagged out of service? *
Training & Inspection
- Have exposed employees been trained on fall and equipment hazards by a qualified person? *
- Are walking-working surfaces regularly and periodically inspected and hazards corrected? *
