Written Program & Administration
- Is there a written respiratory protection program with worksite-specific procedures? *
- Is a suitably trained program administrator designated to oversee the program? *
- Is the program evaluated periodically to ensure it remains effective? *
Hazard Assessment & Selection
- Have airborne contaminants and their exposure levels been evaluated to select respirators? *
- Are only NIOSH-certified respirators used? *
- Are cartridges/canisters selected with an appropriate change-out schedule or end-of-service-life indicator? *
Medical Evaluation & Fit Testing
- Has each user completed a medical evaluation before fit testing and use? *
- Are tight-fitting respirator users fit tested before use and at least annually? *
- Are user seal checks performed each time a respirator is donned? *
- Are facial hair and conditions that interfere with the seal prohibited for tight-fitting respirators? *
Maintenance, Storage & Training
- Are respirators cleaned, disinfected, inspected, and stored properly? *
- Is breathing air for supplied-air/SCBA of the required grade and quality? *
- Is annual training provided on proper use, limitations, and maintenance? *
- Are records of medical evaluations and fit tests retained? *
