Prerequisites
- Has the user received a medical evaluation and clearance before fit testing and use? *
- Is fit testing conducted with the same make, model, style, and size the user will wear? *
- Is the user free of facial hair or conditions that interfere with the face-to-facepiece seal? *
Fit-Test Method
- Is an OSHA-accepted qualitative or quantitative fit-test protocol used? *
- For tight-fitting respirators, is fit testing performed before initial use? *
- Is fit testing repeated at least annually and after facial changes? *
- For quantitative tests, is the required fit factor achieved (100 half-mask / 500 full-face)? *
User Seal Checks & Use
- Are users trained to perform positive- and negative-pressure user seal checks each time they don the respirator? *
- Are voluntary users provided Appendix D information? *
- Are respirators stored, cleaned, and maintained to preserve the seal? *
Records
- Is a fit-test record kept (name, test type, make/model/size, date, results)? *
- Are fit-test records retained until the next fit test is administered? *
- List users due for annual fit-test within 60 days: *
