A pre-startup safety review verifying that new or modified equipment is safe to operate before introducing hazardous energy or process chemicals.

Construction & Installation
- Is construction and equipment verified to be in accordance with design specifications? *
- Are punch-list items affecting safe operation closed or dispositioned before startup? *
- Have pressure tests, leak tests, and functional checks been completed and documented? *
Safety Systems
- Are safety, relief, interlock, and alarm systems installed, tested, and functional? *
- Are emergency shutdown and isolation devices verified operational? *
- Is required guarding, ventilation, and PPE in place for the new equipment? *
Procedures & Training
- Are safety, operating, maintenance, and emergency procedures in place before startup? *
- Have affected employees been trained on the new/modified equipment and procedures? *
- Is lockout/tagout information updated for the new equipment? *
Hazard Review & Authorization
- For new facilities, has a process hazard analysis been performed and recommendations resolved? *
- For modifications, are management-of-change requirements satisfied? *
- Is startup authorized and signed off by the responsible authority? *
