New-Equipment Pre-Startup Safety Review (PSSR)

A pre-startup safety review verifying that new or modified equipment is safe to operate before introducing hazardous energy or process chemicals.

Construction & Installation

  • Is construction and equipment verified to be in accordance with design specifications? *
  • Are punch-list items affecting safe operation closed or dispositioned before startup? *
  • Have pressure tests, leak tests, and functional checks been completed and documented? *

Safety Systems

  • Are safety, relief, interlock, and alarm systems installed, tested, and functional? *
  • Are emergency shutdown and isolation devices verified operational? *
  • Is required guarding, ventilation, and PPE in place for the new equipment? *

Procedures & Training

  • Are safety, operating, maintenance, and emergency procedures in place before startup? *
  • Have affected employees been trained on the new/modified equipment and procedures? *
  • Is lockout/tagout information updated for the new equipment? *

Hazard Review & Authorization

  • For new facilities, has a process hazard analysis been performed and recommendations resolved? *
  • For modifications, are management-of-change requirements satisfied? *
  • Is startup authorized and signed off by the responsible authority? *
  • Record the PSSR completion date and approver: *
