Written Hazard Assessment
- Has a workplace hazard assessment been performed to determine PPE needs? *
- Is the assessment certified in writing identifying the workplace, person certifying, and date? *
- Are engineering and administrative controls considered before relying on PPE? *
Eye, Face & Head Protection
- Is eye/face protection provided where impact, dust, chemical, or optical hazards exist? *
- Are hard hats provided where head-injury hazards exist? *
- Is eye/face protection rated for the specific hazard (impact, splash, radiant energy)? *
Hand, Foot & Body Protection
- Are appropriate hand protection (gloves) selected for chemical, cut, or thermal hazards? *
- Is protective footwear provided where foot hazards exist? *
- Is high-visibility or protective clothing provided where warranted? *
Hearing & Respiratory
- Where noise exceeds action levels, is hearing protection provided under a conservation program? *
