Applicability & Log Maintenance
- Has the establishment confirmed whether it is partially exempt by size (10 or fewer employees) or industry? *
- Is a separate OSHA 300 Log maintained for each establishment expected to operate one year or longer? *
- Are recordable cases entered on the 300 Log within 7 calendar days of receiving information? *
- Is each case classified correctly (death, days away, restricted/transfer, other recordable)? *
Recordability Determinations
- Are recordability decisions based on work-relatedness, new cases, and the general recording criteria? *
- Are first-aid-only cases correctly excluded using the closed first-aid list? *
- Are privacy-concern cases entered without the employee name and tracked confidentially? *
- Are day counts capped at 180 days away/restricted per case? *
301 Incident Reports
- Is an OSHA 301 (or equivalent) incident report completed for each recordable case? *
- Are 301 reports completed within 7 calendar days of learning of the case? *
- Are all records retained for five years following the year they cover? *
300A Summary Posting
- Is the 300A summary certified by a company executive as required? *
- Is the 300A posted from February 1 through April 30 in a conspicuous location? *
- Is the summary posted even if there were no recordable cases (with zeros entered)? *
Electronic Submission
- Has the establishment determined whether it must electronically submit via the ITA? *
- Was the required data submitted by the March 2 deadline? *
- Are severe injuries (fatality within 8 hours; amputation/loss of eye/inpatient hospitalization within 24 hours) reported to OSHA? *
