Permit Coverage & SWPPP
- Has the facility filed a Notice of Intent and obtained NPDES stormwater permit coverage? *
- Is a written SWPPP prepared, kept on site, and current? *
- Does the SWPPP identify the pollution prevention team and industrial activities exposed to stormwater? *
Control Measures & BMPs
- Are good housekeeping and spill prevention/response control measures implemented? *
- Are exposed materials, containers, and waste minimized or covered to prevent contact with stormwater? *
- Is erosion and sediment control maintained on site? *
Monitoring & Sampling
- Are required benchmark and/or effluent monitoring samples collected on schedule? *
- Are outfalls identified and mapped in the SWPPP site map? *
- Are corrective actions taken and documented when benchmarks are exceeded? *
Inspections & Recordkeeping
- Are routine facility inspections conducted at the required frequency? *
- Are quarterly visual assessments of stormwater discharge documented? *
- Are inspection reports, monitoring data, and SWPPP updates retained for at least 3 years? *
