Permit Authorization
- Has a hot work permit been issued and authorized before work begins? *
- Could the work be done in a designated safe area instead, avoiding the permit hazard? *
- Permit number, location, and expiration time: *
Area Preparation
- Are combustibles moved at least 35 feet away or protected with fire-resistant covers? *
- Are floor and wall openings within 35 feet covered to prevent spark travel? *
- Has the atmosphere been tested where flammable vapors could be present? *
- Are appropriate fire extinguishers or hose lines available at the work area? *
Fire Watch & Monitoring
- Is a trained fire watch assigned during hot work where required? *
- Does the fire watch remain for at least 30 minutes after work is completed? *
- Is the fire watch equipped with extinguishing equipment and trained in its use? *
Equipment & Gas Cylinders
- Are compressed gas cylinders secured upright with valve protection caps in place? *
- Are oxygen and fuel-gas cylinders in storage separated by 20 feet or a fire barrier? *
- Are hoses, regulators, and cables inspected and free of damage before use? *
