Noise Exposure Monitoring
- Has noise monitoring been conducted where exposures may equal or exceed an 85 dBA 8-hour TWA? *
- Are affected employees notified of their monitoring results? *
- Is monitoring repeated when changes in production or equipment increase exposures? *
Audiometric Testing
- Is a baseline audiogram obtained within 6 months of first exposure at or above the action level? *
- Are annual audiograms provided and compared to the baseline for standard threshold shifts? *
- Are employees with a standard threshold shift notified within 21 days? *
Hearing Protection & Engineering Controls
- Are hearing protectors made available to all employees exposed at or above 85 dBA? *
- Are hearing protectors mandatory for employees above the 90 dBA PEL or with a threshold shift? *
- Have feasible engineering or administrative controls been evaluated to reduce noise? *
Training & Recordkeeping
- Is annual training provided on noise effects, protector use, and audiometric testing? *
- Are noise exposure records and audiometric records retained for the required periods? *
