Responder Coverage
- Where no infirmary or clinic is in near proximity, are adequate trained first-aid responders available? *
- Is responder coverage provided across all shifts and locations? *
- Is the number of trained responders sufficient for headcount and layout? *
Certification Currency
- Are first-aid, CPR, and AED certifications from a recognized provider (e.g., Red Cross, AHA)? *
- Are certifications current and not expired (typically two-year CPR cycle)? *
- Is a roster maintained showing each responder, certification type, and expiration date? *
- List any responders whose certifications expire within 60 days: *
Supplies & Equipment
- Are adequate first-aid supplies readily available and stocked to the applicable ANSI/ISEA Z308.1 class? *
- Where corrosives are used, are suitable eyewash/quick-drench facilities provided within the work area? *
- Are AEDs (if provided) inspected, with charged batteries and unexpired pads? *
Program Administration
- Is bloodborne pathogen exposure control training provided to designated responders? *
- Are refresher and re-certification schedules tracked and enforced? *
