Classification & Training
- Has the material been properly classified and assigned the correct proper shipping name? *
- Are hazmat employees trained and certified within the last 3 years? *
- Is the hazard class, UN/NA number, and packing group determined? *
Packaging
- Is the material packaged in authorized UN-specification packaging where required? *
- Do package quantities comply with the quantity limits for the mode of transport? *
- Is the packaging closed and secured per the manufacturer's closure instructions? *
Marking, Labeling & Placarding
- Is the package marked with the proper shipping name and UN/NA identification number? *
- Are the correct hazard warning labels applied? *
- Are placards applied to the transport vehicle when required? *
Shipping Papers & Emergency Info
- Do shipping papers include the required basic description in proper sequence? *
- Is a 24-hour emergency response telephone number provided? *
- Is emergency response information (such as an ERG-consistent document) available with the shipment? *
- Is a shipper's certification signed on the shipping paper? *
