Space Evaluation & Permit

  • Has the space been evaluated and classified as a permit-required confined space? *
  • Is a written entry permit completed and signed by the entry supervisor before entry? *
  • Is the permit posted at the entry point and valid only for the stated duration? *
  • Are all known physical, chemical, and engulfment hazards identified on the permit? *

Atmospheric Testing

  • Is the atmosphere tested for oxygen (19.5%-23.5%), flammable gases (<10% LEL), and toxics before entry? *
  • Is the gas monitor calibrated and bump-tested prior to use? *
  • Is continuous or periodic monitoring performed during occupancy? *
  • Record oxygen, LEL, CO, and H2S readings: *

Isolation & Ventilation

  • Are energy sources isolated, blanked, or locked/tagged out before entry? *
  • Is mechanical ventilation provided and directed to the entrant's work area where needed? *
  • Are lines carrying hazardous materials disconnected or double-block-and-bleed applied? *

Roles, Communication & Rescue

  • Are authorized entrants, an attendant, and an entry supervisor assigned and trained? *
  • Does the attendant remain outside and maintain constant communication with entrants? *
  • Is retrieval equipment (full-body harness and mechanical device) in place for vertical entries over 5 feet? *
  • Is rescue capability arranged and able to respond in a timely manner? *
