Exposure Control Plan
- Is a written exposure control plan in place and reviewed at least annually? *
- Does the plan include an exposure determination listing at-risk job classifications and tasks? *
- Does the annual review reflect changes in technology that reduce or eliminate exposure? *
Engineering & Work Practice Controls
- Are sharps with engineered sharps injury protections used and evaluated with frontline input? *
- Are puncture-resistant, labeled sharps containers available and not overfilled? *
- Is recapping of needles by hand prohibited except where required and done one-handed? *
- Are handwashing facilities or antiseptic cleansers readily accessible? *
PPE & Housekeeping
- Is appropriate PPE (gloves, gowns, face protection) provided at no cost to employees? *
- Are surfaces and equipment decontaminated with an appropriate disinfectant after contact? *
- Is regulated waste placed in closable, leak-proof, labeled or color-coded containers? *
Vaccination, Training & Records
- Is hepatitis B vaccination offered at no cost within 10 days of assignment? *
- Is confidential post-exposure evaluation and follow-up provided after an exposure incident? *
