Program & Service Provider
- Is a written IPM program documented, including scope and responsibilities? *
- Is the pest control operator licensed/certified with insurance on file? *
- Are current pesticide labels and SDS available for all products used on site? *
- Is service frequency defined and adhered to per the contract/scope? *
Device Mapping & Monitoring
- Is a current site map showing all bait stations, traps, and monitoring devices maintained? *
- Are exterior rodent bait stations tamper-resistant, anchored, and numbered? *
- Are interior devices non-toxic (mechanical/glue) with rodenticide prohibited inside production? *
- Are insect light traps positioned away from product zones and serviced regularly? *
Exclusion & Sanitation
- Are exterior doors, dock levelers, and wall/floor penetrations sealed against pest entry? *
- Is the exterior perimeter maintained free of harborage, standing water, and vegetation? *
- Are waste and dumpster areas managed to reduce pest attraction? *
Trending, Corrective Action & Records
- Are service reports reviewed and pest activity trended over time? *
- Are corrective actions documented and closed out for pest findings? *
- Is an annual program assessment conducted by the provider or entomologist? *
