Foreign Supplier Verification Program (FSVP) Checklist
Confirm an importer's Foreign Supplier Verification Program identifies hazards, evaluates suppliers, and verifies controls.

Importer & Qualified Individual
- Is the FSVP importer of record identified for each imported food line? *
- Is a qualified individual designated to develop and perform FSVP activities? *
- Is the importer's DUNS number provided in the entry filing for each line? *
Hazard Analysis
- Is a hazard analysis conducted for each food to identify hazards requiring a control? *
- Are biological, chemical (including radiological), and physical hazards evaluated? *
- Is it determined who will control each identified hazard (supplier or downstream)? *
Supplier Evaluation & Verification
- Is each foreign supplier evaluated for its performance and the risk posed by the food? *
- Are appropriate supplier verification activities established and performed? *
- For a SAHCODHA hazard controlled by the supplier, is an annual onsite audit performed (or justified alternative)? *
- Are corrective actions taken when a supplier is found not adequately controlling a hazard? *
Recordkeeping
- Are FSVP records maintained, signed, and available to FDA on request? *
- Are records retained for at least 2 years? *
- Is the FSVP reassessed at least every 3 years or when new information arises? *
