Preoperative Preparation
- Is hair removal avoided, or performed with clippers rather than razors when necessary? *
- Is preoperative bathing/showering with soap or an antiseptic agent performed? *
- Is an alcohol-based skin antiseptic used for skin preparation unless contraindicated? *
Antimicrobial Prophylaxis
- Is prophylactic antibiotic administered within 60 minutes before incision (120 minutes for vancomycin/fluoroquinolones)? *
- Is antibiotic selection consistent with facility guidelines for the procedure type? *
- Is intraoperative redosing performed for prolonged procedures or major blood loss? *
- Is prophylaxis discontinued within 24 hours after surgery end (48 hours for cardiac)? *
Intraoperative Controls
- Is perioperative normothermia maintained? *
- Is perioperative glycemic control maintained (target <200 mg/dL) for all patients? *
- Is increased fraction of inspired oxygen delivered intraoperatively and postoperatively where appropriate?
Surveillance & Feedback
- Are SSIs identified using current NHSN definitions with defined surveillance periods? *
- Is the SSI standardized infection ratio calculated and reported by procedure? *
- Is surgeon-specific SSI data reviewed and shared confidentially with providers?
