Newborn Identification
- Are at least two patient identifiers used, with distinct-naming conventions for newborns to prevent misidentification? *

Human Milk & Feeding Safety

- Are breast milk and formula stored at monitored temperatures with expiration tracking? *
- Is there a documented process for managing a breast milk misadministration event? *
Thermoregulation & Environment
- Are infants maintained in a neutral thermal environment with continuous temperature monitoring? *
- Does the NICU environment support developmental care (light and noise control) per FGI guidance? *
Medication & Equipment Safety
- Are neonatal medications dosed by weight with standardized concentrations and independent double checks? *
- Are alarms on monitors, ventilators, and infusion pumps set to appropriate neonatal parameters and managed to reduce fatigue? *
- Is emergency resuscitation equipment sized for neonates immediately available and checked? *
Level of Care & Staffing
- Is the NICU's level of neonatal care (I-IV) designated consistent with AAP definitions and available services? *
- Are nurse-to-patient ratios matched to acuity per AWHONN/AAP staffing guidelines? *
