Four-Zone Access Control
- Is the MRI environment organized into ACR Zones I-IV with clearly marked boundaries? *
- Is Zone III access physically restricted and supervised so only screened individuals enter? *
- Is Zone IV (the magnet room) accessible only under the direct supervision of MR personnel? *
Screening & Ferromagnetic Control
- Are all patients and non-MR personnel screened with a documented questionnaire before entering Zone III/IV? *
- Is ferromagnetic detection or metal screening used to prevent projectile hazards? *
- Are patients changed into gowns and freed of metallic objects before entering the scanner room where indicated? *
Personnel Roles & Implants
- Has an MR Medical Director and an MR Safety Officer been designated with defined responsibilities? *
- Are implants and devices verified as MR Safe, MR Conditional, or MR Unsafe with conditions honored before scanning? *
- Are Level 1 and Level 2 MR personnel trained and their status documented? *
Contrast, Emergencies & Quench
- Is renal function/eGFR screened before gadolinium-based contrast where clinically indicated? *
- Is a documented procedure for MR-related emergencies, including patient removal from Zone IV, in place? *
- Are quench procedures and oxygen monitoring in place for cryogen release, with staff trained? *
