IRF Classification & 60% Rule
- Does at least 60% of the facility's inpatient population require intensive rehabilitation for one of the qualifying CMS conditions? *
- Is compliance with the 60% threshold monitored and documented across the applicable review period? *
Preadmission Screening & Admission
- Is a comprehensive preadmission screening completed within 48 hours prior to admission and used to determine appropriateness? *
- Is a post-admission physician evaluation completed within 24 hours of admission? *
- Is an individualized overall plan of care completed by a rehabilitation physician within 4 days of admission? *
Intensity of Therapy
- Does the patient require and receive intensive therapy, generally interpreted as at least 3 hours per day, 5 days per week (or 15 hours over 7 days)? *
- Does the patient require at least two therapy disciplines, one of which is physical or occupational therapy? *
- Is close physician supervision documented, with rehabilitation physician face-to-face visits at least 3 days per week? *
Interdisciplinary Team & Conferences
- Does the interdisciplinary team include a rehabilitation physician, rehabilitation nurse, and relevant therapists? *
- Are interdisciplinary team conferences held at least once per week to assess progress toward goals? *
IRF-PAI & CARF Program Standards
- Is the IRF Patient Assessment Instrument (IRF-PAI) completed and transmitted for each Medicare patient within required timeframes? *
- Are measurable, person-centered rehabilitation goals established with the person served and documented per CARF standards? *
- Does the program collect and analyze outcomes data (effectiveness, efficiency, satisfaction) for performance improvement? *
