Patient Rights & Admission
- Is each patient provided written notice of their rights before care is initiated? *
- Is the patient informed of the plan of care, expected outcomes, and any changes in advance? *
- Is a process in place to investigate complaints and communicate resolution to the patient? *
Comprehensive Assessment
- Is a comprehensive assessment completed within 5 days after the start of care? *
- Does the assessment incorporate the OASIS data set for applicable patients? *
- Is the comprehensive assessment updated at required intervals and upon significant change? *
- Is a drug regimen review performed to identify potential adverse effects and interactions? *
Plan of Care & Coordination
- Is an individualized plan of care established and reviewed by the physician/allowed practitioner? *
- Are verbal orders documented, dated, and signed by the physician per policy? *
- Is care coordinated among all disciplines with communication of changes in the patient's condition? *
QAPI & Personnel
- Does the agency operate a data-driven QAPI program measuring and improving outcomes? *
- Are home health aides competency-evaluated and supervised at the required frequency? *
- Are clinical records maintained and retained for the required period with safeguards for confidentiality? *
