General Safety

  • Are corridors and egress paths free of clutter, unattended equipment, and tripping hazards? *
  • Are floors dry, and are wet-floor/spill hazards addressed with signage and prompt cleanup? *
  • Are ceiling tiles intact and free of water stains indicating leaks or contamination? *
  • Are electrical outlets, cords, and equipment free of damage with no unapproved power strips? *

Security & Environment

  • Are sensitive/high-risk areas (e.g., pharmacy, nursery, behavioral health) access-controlled? *
  • Is emergency call/duress notification functioning in patient care and isolated work areas? *
  • Are infant/vulnerable patient security systems (if applicable) tested and functional?

Hazardous Materials & Waste

  • Are hazardous chemicals labeled and are Safety Data Sheets accessible to staff? *
  • Is regulated medical (biohazard) waste segregated, labeled, and stored appropriately? *
  • Are sharps containers replaced before the fill line and mounted securely? *
  • Are compressed gas cylinders secured, segregated (full/empty), and stored upright? *

Utility Systems

  • Are emergency power outlets (red receptacles) clearly identified and unobstructed? *
  • Are medical gas shutoff (zone) valves labeled, accessible, and unobstructed? *
  • Are temperature, humidity, and air-pressure relationships maintained in critical areas (OR, sterile)? *

Medical Equipment

  • Is medical equipment inventoried and inspected/tested per the equipment management plan? *
  • Do devices in use display current preventive-maintenance/inspection tags? *
  • Are recalled or malfunctioning devices removed from service and reported per policy? *
