Performance & Documentation

  • Is high-quality CPR (rate, depth, minimal interruptions) and defibrillation performed per AHA guidelines? *
  • Is a complete code record with timed interventions and medications documented? *
  • Do responders maintain current BLS/ACLS certification as required for their role? *

Review & Improvement

  • Is a post-event debrief conducted after code and rapid response events? *
  • Are code and rapid response events and outcomes reviewed by a resuscitation committee? *
  • Are time-to-defibrillation and survival-to-discharge metrics tracked?
