Resident Rights & Dignity
- Is each resident treated with dignity and respect and free from abuse, neglect, and exploitation? *
- Is each resident informed of their rights and facility rules before or at the time of admission? *
- Can residents self-administer medications when the interdisciplinary team deems it safe? *
- Are residents allowed to participate in developing their own care plan? *
Comprehensive Assessment & Care Planning
- Is a comprehensive MDS assessment completed within 14 days of admission and updated on schedule? *
- Does each resident have a person-centered, baseline care plan within 48 hours of admission? *
- Is a comprehensive care plan developed within 7 days of the comprehensive assessment? *
Quality of Care & Quality of Life
- Do residents receive care to maintain/improve function unless clinically contraindicated? *
- Are residents free of avoidable pressure ulcers and are existing ulcers treated? *
- Are interventions in place to prevent avoidable accidents and falls (adequate supervision/devices)? *
- Is each resident free from unnecessary physical restraints? *
Nursing Services & Staffing
- Are sufficient nursing staff provided to meet residents' assessed needs 24 hours a day? *
- Is a registered nurse on duty at least 8 consecutive hours per day, 7 days a week? *
- Are nurse aides competency-evaluated and included on the state nurse aide registry? *
Pharmacy & Medication Management
- Is the drug regimen of each resident reviewed at least monthly by a licensed pharmacist? *
- Is each resident free from unnecessary drugs and any excessive dose/duration? *
- Are psychotropic drugs given only on a PRN basis for a limited duration with documented rationale? *
Food, Nutrition & Environment
- Are foods prepared, stored, distributed, and served under sanitary conditions? *
- Does the facility maintain a safe, functional, sanitary, and comfortable environment? *
- Is there a facility assessment used to determine resource and staffing needs? *
