Insertion Bundle
- Was hand hygiene performed immediately before catheter insertion? *
- Were maximal sterile barrier precautions used (cap, mask, sterile gown, sterile gloves, full-body drape)? *
- Was the skin prepped with a >0.5% chlorhexidine/alcohol preparation and allowed to dry? *
- Was the subclavian site preferred over femoral for non-tunneled catheters in adults when feasible? *
- Was a central line insertion checklist completed with an empowered observer able to stop the procedure? *
Maintenance Bundle
- Is the daily necessity of the central line assessed and documented, with prompt removal when no longer needed? *
- Are catheter hubs, needleless connectors, and injection ports scrubbed before each access? *
- Are transparent dressings changed every 7 days (or sooner if soiled/loose) and gauze every 2 days? *
- Is a chlorhexidine-impregnated dressing or sponge used at the insertion site where indicated?
Surveillance & Response
- Are CLABSI events identified and reported using the current NHSN surveillance definitions? *
- Is the standardized infection ratio (SIR) tracked and reviewed by unit? *
- Is a root-cause analysis or apparent-cause review completed for each CLABSI? *
Education & Competency
- Are staff who insert or maintain central lines trained and competency-assessed on the bundle? *
- Is patient/family education on central line care provided where appropriate?
- Is bundle compliance feedback provided to frontline staff on a regular basis? *
