Appropriate Use

  • Was the urinary catheter placed only for a documented, approved indication? *
  • Were alternatives to indwelling catheterization (e.g., condom catheter, intermittent catheterization, bladder scan) considered? *
  • Is the ongoing need for the catheter reviewed daily with a nurse-driven removal protocol? *

Insertion Technique

  • Was the catheter inserted using aseptic technique and sterile equipment by trained personnel? *
  • Was hand hygiene performed immediately before and after insertion or manipulation? *
  • Was the smallest bore catheter possible selected and properly secured after insertion? *

Maintenance & Drainage

  • Is a closed sterile drainage system maintained without breaks? *
  • Is the drainage bag kept below the level of the bladder and off the floor at all times? *
  • Is unobstructed urine flow maintained without dependent loops or kinks in the tubing? *

Surveillance

  • Are CAUTI events identified using current NHSN surveillance definitions? *
  • Are catheter utilization ratios and CAUTI SIR tracked by unit? *
  • Is feedback on device days and infection rates provided to frontline staff?
