Exposure Control Plan

  • Is a written Exposure Control Plan in place, accessible to employees, and reviewed and updated at least annually? *
  • Does the plan include an exposure determination listing job classifications with occupational exposure? *
  • Does the annual review reflect changes in technology that reduce or eliminate exposure and solicit input from frontline workers? *

Engineering & Work Practice Controls

  • Are sharps with engineered sharps injury protections (safety-engineered devices) used where feasible? *
  • Are puncture-resistant, labeled sharps containers available at point of use and not overfilled? *
  • Is a sharps injury log maintained recording device type, department, and circumstances of each injury? *
  • Is recapping of needles by hand prohibited except where no alternative is feasible? *

Vaccination, PPE & Labeling

  • Is hepatitis B vaccination offered at no cost within 10 working days of assignment to employees with occupational exposure? *
  • Is appropriate PPE (gloves, gowns, face protection) provided at no cost and used? *
  • Are biohazard labels or red containers used for regulated waste, contaminated laundry, and specimens? *

Training & Post-Exposure Follow-Up

  • Is bloodborne pathogens training provided at initial assignment and at least annually thereafter? *
  • Is a confidential post-exposure medical evaluation, source testing, and follow-up provided at no cost after an exposure incident? *
  • Are medical and training records retained for the required durations (medical: duration of employment plus 30 years)? *
