Governing Body & Administration
- Does the ASC have a governing body legally responsible for the conduct of the ASC? *
- Has the ASC entered into a written transfer agreement with a local hospital, or do physicians have admitting privileges? *
- Does the governing body ensure a QAPI program is ongoing and data-driven? *
Patient Rights & Admission
- Are patients informed of their rights, advance directive policy, and grievance process before surgery? *
- Is a comprehensive medical history and physical assessment completed and updated before surgery? *
- Is the patient assessed for risks and appropriateness for the ambulatory setting? *
Surgical & Anesthetic Services
- Are surgical procedures performed only by qualified physicians granted privileges by the governing body? *
- Is a physician present to evaluate the patient before discharge or is discharge per approved criteria? *
