Permit Application
- Is the encroachment/ROW permit application complete with legal description and limits of work? *
- Are approved plans and traffic-control plans attached? *
- Is proof of contractor licensing and insurance provided? *
- Is a surety/restoration bond or deposit posted as required? *
Utility Connections & Will-Serve
- Are will-serve / availability letters obtained from water, sewer, power, gas, and telecom providers? *
- Are tap/connection sizes and locations approved by each utility? *
- Are backflow prevention and metering requirements coordinated? *
- Are easements or dedications recorded for private utility extensions? *
Underground Locates & Safety
- Has a one-call/811 locate ticket been requested before excavation? *
- Are existing utilities positively located (potholed) at crossings? *
- Is a competent-person excavation/trench safety plan in place? *
Traffic Control & Access
- Is temporary traffic control (signage, cones, flaggers) deployed per the approved plan? *
- Are pedestrian access and ADA-compliant temporary routes maintained? *
- Are lane closures scheduled within permitted hours? *
Restoration & Closeout
- Are trench backfill, compaction, and pavement restoration performed to agency standard? *
- Is final restoration inspected and accepted by the agency? *
- Is the permit closed and bond/deposit released? *
