Company & Legal Standing
- Is a completed prequalification statement (AIA A305 or equivalent) on file? *
- Are trade licenses and business registrations current for the project jurisdiction? *
- Is there current litigation, liens, or defaults disclosed?
- How many years has the firm operated under its current name?
Financial Capacity & Bonding
- Have audited or reviewed financial statements been provided? *
- Is the single-project and aggregate bonding capacity adequate for this scope? *
- Is a letter of bondability from the surety provided? *
- Is the subcontractor's largest completed project value comparable to this scope? *
Safety Record
- Is the current Experience Modification Rate (EMR) at or below the threshold?
- Have OSHA 300 logs / TRIR and DART rates been reviewed? *
- Is there a written safety program and designated safety officer? *
- Are there any willful/serious OSHA citations in the last 3 years disclosed? *
Capacity, Quality & References
- Is the current backlog/workload reviewed against available capacity for this project? *
- Have project references been contacted and rated?
- Is a quality management or QA/QC program documented? *
- Prequalification approval decision *
