A verification checklist for confirming a subcontractor's certificate of insurance meets contract requirements before allowing work to begin on site.

Certificate Basics
- Is a current ACORD 25 certificate on file with the correct named insured (matching the subcontract)? *
- Are all policies in effect (not expired) for the anticipated work period? *
- Is the certificate holder listed correctly as the general contractor/owner? *
- Are carrier AM Best ratings acceptable (e.g., A- VII or better)? *
General Liability
- Do the per-occurrence and aggregate limits meet or exceed the contract minimum? *
- Is the GC/owner named as additional insured on a primary and non-contributory basis? *
- Does the additional insured include ongoing AND completed operations? *
- Is a waiver of subrogation in favor of the GC/owner provided? *
- Are exclusions (residential, EIFS, subsidence) reviewed against the scope? *
Workers Compensation & Auto
- Is workers compensation coverage at statutory limits with employers liability? *
- Is a waiver of subrogation included on the workers comp policy? *
- Is commercial auto liability provided at the required combined single limit? *
Excess & Records
- Does umbrella/excess coverage follow-form over GL, auto, and employers liability to meet limits? *
- Are actual endorsement forms (not just the certificate) collected for verification? *
- Is a renewal tracking/tickler in place before expiration? *
- COI approved for site access *
