Product Qualification & Documents
- Is a current evaluation report (ICC-ES ESR) on site covering the specified anchor? *
- Does the anchor type, diameter, and embedment match the approved drawings? *
- Are the manufacturer's printed installation instructions (MPII) available at the work area? *
- For adhesive anchors, is the installer certified (ACI/CRSI) where required? *
Hole Preparation
- Is the drilled hole diameter and depth per the MPII for the anchor size? *
- Is the drilling method (hammer/core) consistent with the qualification? *
- Has the hole been cleaned (brush/blow) per the required cycle count? *
- Is edge distance and anchor spacing within the approved minimums? *
Installation
- For adhesive anchors, is the adhesive within its shelf-life and above minimum base-material temperature? *
- Is adhesive injected from the bottom of the hole with no air voids? *
- Is the required gel/cure time observed before loading? *
- For mechanical anchors, is the installation torque verified with a calibrated wrench? *
- Is the anchor embedment depth verified and marked? *
Verification & Testing
- Is periodic or continuous special inspection provided per the anchor category? *
- Where specified, has proof/tension testing been performed to the required load? *
- Are inspection records and any nonconformances documented? *
- Photo of installed anchors and hole preparation
